JUDIT, a young Puerto Rican woman, wakes up inside an isothermal airtight bag. She doesn't recall how she got there. What she knows for sure is that she is not dead. There's a strange tattoo on her neck. It wasn't there before. It looks like a barcode. She reaches inside the bag and pulls out a QR code scanner, a tablet and a syringe containing a colored liquid. After managing to get herself out of the bag, she looks around and concludes that she's in some kind of crematorium. There's a disturbing, crackling noise coming from outside. She looks out a tiny window and sees that the building is surrounded by flames. Everything out there seems to be burning. She manages to access her phone and finds out that her city is under a major bacteriological attack. Something stirs behind her. JOEL, a middle-aged white man is staring at her. The airtight bag he was in lays at his feet. Like Judit, he has no clue how he got there. They agree they'll figure it out later. First thing to do is leave this place before it burns down to ashes. They will also soon discover that leaving the building alive is the least of their problems. Joel has alzheimer. And, alas, he's also a serial killer.

But don Carmine has been patiently waiting for John's release, hatching his revenge for 18 years. Don Carmine's men kidnap John and his daughter. They torture him and kill her before his very eyes. John barely survives. When he eventually recovers his sole reason to live is go after Don Carmine, and thus sets out for his brutal revenge.
